What is Artefill?

ArteFill consists of polymethylmethacrylate (PMMA) microspheres (20% by volume), 30–50 μm in diameter, suspended in 3.5% bovine collagen solution (80% by volume) and 0.3% lidocaine.

What is Artecoll? 
Artecoll contains 20% PMMA Microspheres (30-40 microns in diameter) and 80% bovine collagen, along with 0.3% lidocaine. 

What is Bellafill? 
Bellafill is an implant filler composed of non-resorbable polymethylmethacrylate (PMMA) microspheres, 30 to 50 microns in diameter, suspended in a water based carrier gel composed of 3.5% bovine collagen, 92.6% buffered, isotonic water for injection, 0.3% lidocaine hydrochloride, 2.7% phosphate buffer, and 0.9% sodium chloride. Bellafill seems to have the ability to either migrate locally or to cause local inflammatory conditions that may lead to chronic swelling and nodules. Expectations on the Day of Surgery for Bellafill Nodules or Granuloma
 
Patients who have under eye, lip, or facial Bellafill swelling and nodules may benefit from a procedure in which the Bellafill is surgically removed or dissolved partially. The most important nonsurgical option is the introduction of intralesional injections in order to soften the scar tissue around the Bellafill, a surgical option for the eyelids includes a subciliary incision whereas for the lips, a mucosal incision, followed by surgical extirpation and removal of the unwanted Bellafill that is causing problems. Not everyone is a surgical candidate, but surgery continues to be a safe and affordable option for many patients. 

PMMA Removal from the body may be a difficult prospect, and much different than silicone biopolymer removal. Outpatient Surgery for Bellafill Removal 

Your Bellafill PMMA removal surgery is usually performed as an outpatient technique with either local anesthesia or sedation. Major procedures of the face or body may require a more invasive technique with the support of general anesthesia. Rest assured that Dr. Karamanoukian and his team will work closely with the anesthesiologist, if anesthesia is required, in order to safeguard your health during the procedure. Surgical extirpation of PMMA and Bellafill involves the removal of filler with small, well-hidden incisions placed in the skin.

Recovery After Bellafill Removal

On recovery after your surgery, you will be advised to avoid strenuous activity or movements that may compromise your incision line and scar repair. Patients usually recover quickly, but this all depends on their individual health, the status of the tissue, and the severity of the surgery. Most patients will recover within one to two weeks of their surgery, in which time they can begin to engage in normal social activities without hesitation. 

Our Kare Plastic Surgery patients report that silicone biopolymer or PMMA surgery is usually tolerable in terms of pain. During surgery, your tissue will be anesthetized with either local anesthesia or tumescent. Post-treatment, patients report minimal pain within one to two days after surgery. Results are seen immediately, but are often obscured by swelling and edema after surgery. Within one to two weeks after your PMMA removal, you will begin to see the contours of the reconstructive treatments. 

Risks associated with the surgery are specific to your treatment area, but may include bruising, swelling, silicone embolization, migration of silicone and PMMA, foreign body tissue reaction, autoimmune disease, pulmonary embolism, worsening of your health, and longterm debility. Unfortunately, many of these complications are a direct result of the permanent filler and may be exacerbated by surgical removal. In other words, permanent fillers may have longterm consequences without treatment. Treating the fillers may only unmask these naturally-occuring complications. We have found, through our clinical research and histological case reviews, that the silicone dermal fillers used throughout Colombia, Mexico, and South America are often undesirable copies of Silikon 1000 that may cause longterm inflammatory complications. Foreign body tissue reaction and granulomas are common among these fillers.
