The Timing Science
When Should Face Stitches Come Out?
The single most important variable in whether ER stitches produce a good scar or a visible one is not the quality of the closure — it is whether the sutures are removed on time. Non-absorbable sutures (nylon, prolene, silk) left in the skin beyond their optimal removal window allow the skin to grow a tunnel of epithelium around each suture — producing the perpendicular rail marks on either side of the wound that constitute suture track scarring, also called railroad scarring. The optimal removal timing is determined by the anatomical location’s healing rate and the mechanical tension on the wound:
| Location |
Optimal Removal Window |
Risk of Track Scarring If Late |
Post-Removal Protocol |
| Face (eyelid, lip, nose, cheek) |
5-10 days |
Very high — permanent marks by day 7 |
Steri-strip, silicone gel, SPF 50, Vbeam at 4 wks |
| Forehead |
5–10 days |
High — visible tracks by day 10 |
Silicone tape, Vbeam, sunscreen strict |
| Scalp |
5–7 days |
Moderate — hidden by hair but scar widening risk |
Silicone gel, hair protection, no heat tools |
| Hand / fingers |
10 days |
Moderate — high-tension site, risk of dehiscence if early |
Silicone tape, joint protection during healing |
| Trunk / back / extremity |
10 days |
Lower but present — body scar widening risk |
Silicone tape, tension management |
| Foot / sole |
14–21 days |
Low track risk, high dehiscence risk if early |
Offloading, wound check at removal |
“Once stitches are placed in the Emergency Room, it is imperative that a plastic surgeon manage the stitches and post-op wound care so the scar appears seamless.”

Can I remove my own ER stitches at home?
Self-removal of simple, accessible non-absorbable sutures on body locations (not face, hands, or genitalia) is technically possible but is not recommended without a physician assessment. For facial sutures specifically, self-removal is not appropriate due to the precision required and the consequences of removing too early or leaving any suture fragment. Call (310) 998-5533 for a same-day appointment.
What is the difference between absorbable and non-absorbable stitches from the ER?
Absorbable sutures (Vicryl, chromic gut, Monocryl) dissolve within the tissue over days to weeks and do not require removal — they are commonly used for deep internal layers and sometimes for scalp closures. Non-absorbable sutures (nylon, prolene, silk) remain in the skin indefinitely until removed — they are used for the final skin closure on most lacerations and all require follow-up removal at the correct interval. The ER discharge paperwork should specify the suture material and recommended removal timing. If it does not, call (310) 998-5533 for a same-day assessment and Dr. Karamanoukian will evaluate the wound and determine the appropriate removal timing.
