PAH typically becomes apparent 2 to 6 months after the CoolSculpting procedure, not immediately after. Patients initially notice a gradual fullness returning to the treated area that continues to grow rather than resolve over the subsequent months.
PAH Treatment — Liposuction Is the Only Effective Solution
PAH does not resolve on its own. No non-surgical treatment — including additional CoolSculpting, radiofrequency devices, massage, or diet — removes PAH fat tissue. Liposuction is the only intervention that effectively corrects the condition and restores the body contour. However, the timing of surgery matters critically.
The PAH fat tissue is too firm and fibrous to remove safely immediately after it develops. Patients must wait 6 to 9 months from the time the PAH becomes apparent for the tissue to soften adequately. During this waiting period, Dr. Karamanoukian monitors the patient and plans the liposuction approach based on the volume and location of the PAH deposit. For abdominal PAH with associated skin laxity, combining liposuction with abdominoplasty at the same surgery addresses both the fat volume and the skin quality in a single procedure.
VASER liposuction for PAH: Because PAH fat is denser and more fibrous than normal subcutaneous fat, Dr. Karamanoukian uses VASER ultrasound-assisted liposuction for many PAH cases. The VASER probe emits ultrasound energy that selectively emulsifies the dense PAH fat before aspiration, making removal more complete and reducing trauma to the surrounding tissue. The result is a smoother contour and more complete fat removal from the fibrous PAH deposit compared to standard suction-assisted liposuction alone.

Who is most at risk for developing PAH after CoolSculpting?
Men are at higher risk for PAH than women. Abdominal CoolSculpting treatments have the highest PAH incidence among treatment areas. Repeated CoolSculpting sessions in the same area also appear to increase PAH risk. The genetic and immunological factors that determine individual susceptibility are not fully understood, which is why PAH cannot be reliably predicted before treatment. Studies now suggest that PAH may occur in approximately 1 out of every 50 CoolSculpting treatment sessions, substantially higher than the 0.005% rate originally reported by the device manufacturer.
Can the medspa that caused my PAH treat it?
PAH is a surgical condition that requires board-certified plastic surgery correction. Medspas are not licensed to perform liposuction and cannot treat PAH.
