Your CoolSculpting Made Things Worse. That Is a Recognized Medical Complication. It Is Treatable.

You went to a medspa for CoolSculpting to reduce fat. Instead, the treated area grew larger. The fat became firm and hard, shaped like the applicator that was placed on your skin. It does not improve with dieting. It does not shrink with exercise. And no amount of additional CoolSculpting can fix it — in fact, more treatment may make it worse. What you have is paradoxical adipose hyperplasia, or PAH — a recognized complication of cryolipolysis in which the cold treatment triggers fat cell expansion rather than destruction. It is frustrating, emotionally distressing, and entirely treatable. But it requires a board-certified plastic surgeon, not the medspa that caused it.

At Kare Plastic Surgery and Skin Health Center at 804 7th Street in Santa Monica near Montana Avenue, UCLA School of Medicine graduate and dual board-certified plastic surgeon Dr. Raffy Karamanoukian, MD, FACS performs PAH correction liposuction for patients across Los Angeles who developed paradoxical adipose hyperplasia after CoolSculpting treatments. He is one of the few surgeons in Los Angeles with documented intraoperative PAH aspirate photography — the dense, fibrous, orange-gold fat emulsion visible in the surgical image below that characterizes PAH tissue at the time of removal and is distinct from the appearance of normal liposuction aspirate.

What PAH Fat Looks Like

Normal liposuction aspirate is a pale yellow liquid fat that flows easily through suction cannulas. PAH aspirate — shown in the collection canister above — is denser, more fibrous, and produces the characteristic orange-gold emulsion visible as the upper layer separated by a sharp interface from the infusion fluid below. This density is why PAH fat requires the softening period of 6 to 9 months before it can be safely and completely removed.

Why PAH Fat Is Different

PAH produces hyperplastic adipose tissue — fat cells that have enlarged and multiplied in response to the cold injury rather than dying as intended. The resulting tissue is firmer, more fibrous, and more vascular than normal subcutaneous fat.

The PAH Correction Result

Once the PAH fat tissue has been completely removed by liposuction, patients achieve the flat, contoured result that CoolSculpting originally promised. In most cases the body contour returns to close to the pre-CoolSculpting baseline or better, depending on the amount of PAH tissue that developed and the quality of the liposuction performed. Dr. Karamanoukian discusses expected outcomes at consultation based on the PAH presentation.

Five Signs You May Have Developed PAH After CoolSculpting

increased fat

The definitive sign of PAH: the area treated with CoolSculpting is now larger than it was before treatment. Not mildly larger. Noticeably, progressively larger. This is the opposite of the expected outcome and cannot be explained by weight gain.

increased density

PAH fat feels different from normal subcutaneous fat. It is firm, rubbery, and denser than the surrounding tissue. Patients frequently describe it as feeling "hard" or "like a brick" under the skin — the fibrous character that the intraoperative image above demonstrates.

The Shape Mirrors the Applicator

A strong diagnostic indicator of PAH is when the enlarged area follows the exact footprint of the CoolSculpting applicator that was placed on the skin — a rectangular or oval bulge that precisely matches the treatment zone.

no resolution

PAH fat does not shrink with dieting, exercise, or additional CoolSculpting treatments. If anything, repeat cryolipolysis may worsen the PAH response. If your treated area has not improved or has worsened at 4 to 6 weeks after treatment, seek evaluation promptly.

It Developed Gradually

PAH typically becomes apparent 2 to 6 months after the CoolSculpting procedure, not immediately after. Patients initially notice a gradual fullness returning to the treated area that continues to grow rather than resolve over the subsequent months.

PAH Treatment — Liposuction Is the Only Effective Solution

PAH does not resolve on its own. No non-surgical treatment — including additional CoolSculpting, radiofrequency devices, massage, or diet — removes PAH fat tissue. Liposuction is the only intervention that effectively corrects the condition and restores the body contour. However, the timing of surgery matters critically.

The PAH fat tissue is too firm and fibrous to remove safely immediately after it develops. Patients must wait 6 to 9 months from the time the PAH becomes apparent for the tissue to soften adequately. During this waiting period, Dr. Karamanoukian monitors the patient and plans the liposuction approach based on the volume and location of the PAH deposit. For abdominal PAH with associated skin laxity, combining liposuction with abdominoplasty at the same surgery addresses both the fat volume and the skin quality in a single procedure.

VASER liposuction for PAH: Because PAH fat is denser and more fibrous than normal subcutaneous fat, Dr. Karamanoukian uses VASER ultrasound-assisted liposuction for many PAH cases. The VASER probe emits ultrasound energy that selectively emulsifies the dense PAH fat before aspiration, making removal more complete and reducing trauma to the surrounding tissue. The result is a smoother contour and more complete fat removal from the fibrous PAH deposit compared to standard suction-assisted liposuction alone.

"PAH patients come to me after months of watching the area they paid to reduce get progressively larger. By the time I see them, the emotional toll is real — they feel embarrassed, angry, and often told by the medspa that it will resolve on its own. It will not. Liposuction corrects it, completely, and the intraoperative photography on this page shows exactly what we remove when we do it."

Frequently Asked Questions

Where can I get PAH correction in Los Angeles?

Kare Plastic Surgery at 804 7th Street in Santa Monica near Montana Avenue offers expert paradoxical adipose hyperplasia correction by UCLA-trained dual board-certified plastic surgeon Dr. Raffy Karamanoukian. CoolSculpting PAH treated with liposuction and VASER after the required 6 to 9 month softening period. Intraoperative PAH aspirate documentation. Call (310) 998-5533. We serve patients from Santa Monica, Beverly Hills, Brentwood, Malibu, and all of Los Angeles.

Who is most at risk for developing PAH after CoolSculpting?

Men are at higher risk for PAH than women. Abdominal CoolSculpting treatments have the highest PAH incidence among treatment areas. Repeated CoolSculpting sessions in the same area also appear to increase PAH risk. The genetic and immunological factors that determine individual susceptibility are not fully understood, which is why PAH cannot be reliably predicted before treatment. Studies now suggest that PAH may occur in approximately 1 out of every 50 CoolSculpting treatment sessions, substantially higher than the 0.005% rate originally reported by the device manufacturer.

Can the medspa that caused my PAH treat it?

PAH is a surgical condition that requires board-certified plastic surgery correction. Medspas are not licensed to perform liposuction and cannot treat PAH. The medspa that performed the CoolSculpting cannot correct the PAH they caused. Dr. Karamanoukian evaluates PAH patients referred from any practice and provides an honest assessment of the extent of PAH, the expected correction achievable with liposuction, and the optimal timing and technique for your specific situation.
