Hyaluronidase dissolves hyaluronic acid fillers quickly and effectively. But not all fillers are HA-based, and not all HA filler complications respond completely to enzyme. Eyelid Filler Excision
Filler placed in the tear trough or lower eyelid that has migrated, calcified, or produced a visible nodule is removed through a transconjunctival incision placed inside the lower eyelid, leaving no visible scar on the skin surface. The eyelid approach requires surgical experience in periorbital anatomy to protect the eyelid margin, tear drainage, and orbital structures. This is the most delicate area of the face for filler excision.
Nose and Nasal Tip Filler Excision
Permanent or non-dissolvable filler in the nose or nasal tip is removed through small incisions placed in natural nasal skin folds or via an open rhinoplasty approach for more extensive deposits. The nose is a high-risk area for filler because of its rich vascular supply and the proximity of the nasal arteries to common injection sites. Preoperative ultrasound mapping is essential before surgical planning in this area.
Lip Filler Excision
Overfilled, migrated, or permanent filler in the lips is removed through an incision placed at the dry-wet junction of the lip, inside the vermilion border. This approach gives direct access to the lip body where filler most commonly accumulates and leaves no visible external scar. Lip filler excision is performed under local anesthesia as an office procedure. It is the preferred approach for silicone, PMMA, or highly cross-linked HA filler that does not respond fully to hyaluronidase.
Chin, Cheek, and Jawline Excision
Large filler deposits in the chin and jawline, particularly PMMA or silicone, are removed through small incisions placed under the chin or inside the mouth in the gingival sulcus. Cheek filler deposits are accessed through intraoral incisions when possible, avoiding visible facial scars. Preoperative ultrasound maps the filler depth and proximity to the facial nerve and its branches before any incision is planned.
Which filler types require surgical removal? Hyaluronidase dissolves only hyaluronic acid fillers. PMMA (Bellafill), liquid silicone, Sculptra, Radiesse, and other biopolymers cannot be dissolved and require surgical excision. Highly cross-linked or calcified HA filler deposits that no longer respond to enzyme also benefit from surgical removal.
